QR Code that employees scan at the job site that takes them to their form

Email Notifications to Heath & Safety Supervisors on any 'no I'm not healthy' check-ins
Reporting: who is completing forms (daily, weekly, monthly)
Visitor Check-in Form: contact tracing info & health check for non-employees
Customization: we can likely build any custom functionality that you require.

Forms start at $500, so this is pretty affordable.
